Undertaking this procedure might results difficult due to the lack of bone and soft tissue in the areas where the implants have to be placed in a correct prosthetic position. The use of zygomatic implants, already used for atrophic jaws with a high success rate, was proposed as alternative to bone grafting also in oncologic patients. The use of computer-aided technologies could improve this method.Aim: This abstract reports the preliminary results of a 3 years follow-up of a prospective study aimed to evaluate the use of zygomatic implants combined with a computer assisted procedure for oncologic patients rehabilitation.Material and methods: Starting from October 2013, seven patients with a defect in upper jaw due to resection for cancer were recruited in our protocol following the ethical committee approval.All the patients were scheduled and treated according with the defect based classification made by Pellegrino and al. (2015). The mean follow-up period was 28 months with a range from 12 to 41 months. The patient age ranged from 47 to 83 years. The mean age of the patients was 64 years. Five patients of class II were treated, one of class I, and one of class III. The total number of Zygomatic/oncologic implants positioned was 23. For all the patients three-dimensional virtual planning was adopted to choose the implant length and position. In five cases a dynamic navigation system was used to reproduce the virtual planning intra-operatively. In five cases an immediate loading was performed. The length of the implants varied from 27 to 52.5 mm according to the post-resection residual anatomy. Clinical and radiographic outcomes were evaluated as well as the major and minor complication. The correspondence between the planned implant length and the postoperative one was evaluated too. Quality of life questionnaire was administered pre-operatively and after the prosthetic rehabilitation.Results: No major complications were recorded. The implant success rate was 95.6% with 100% for the prosthesis. One implant failure was reported until now which did not compromise the prosthesis. The matching between the planned and the real implant length found a good correspondence in the 75.6% of the cases. The quality of life score value was significantly increased with an improvement of 65.5%. The minor complications were two transient mucositis, one broken screw and one prosthesis chipping.Conclusions and clinical implications: The employment of zygomatic implants seems to allow the surgeons the possibility to insert these implants even in case of total maxillary bone defect thanks to the bi-cortical stability they can obtain through the malar bone. The early prosthesis loading certainly allows the patient to gain better functional outcomes. Computer-aided surgery and intra-operative navigation seems to be a useful method to simplify the procedure and to increase accuracy and predictability. More clinical trials with a longer follow-up period are required to confirm the reliability of this method.
